  • I am registering for a program at Shawntay’s School of Creative Nails or Shawntay’s School of Esthetics.

  • I am 17 years of age or older.

  • I have a valid State Identification Card and Social Security Card to verify my identity.

  • I have obtained a High School Diploma or GED, and I can provide documentation upon request.

  • I certify that all information provided during registration is accurate and truthful. I understand that providing false information may result in denial or termination of enrollment.

  • I agree to follow all school policies, procedures, and requirements throughout the duration of my enrollment.
